This form is for parents of children enrolled in the before and/or after school program at a RisingOaks Early Learning centre attached to a Catholic school.  Families are asked to provide information to confirm their plans for the upcoming Spring Break (April 12 - 16).

SUBMISSION DEADLINE
All Opt-out Requests for children regularly scheduled to attend before and/or after school care on these days MUST BE RECEIVED BY MARCH 29th AT 11:59 PM to receive credit for these day.

In addition, using this same form, all Opt-IN Requests for children who do not regularly attend on these days but require care during the Spring Break must be received by MARCH 29th AT 11:59 PM to guarantee care.

NEW: Due to COVID-19, only those registered in RisingOaks' before and after school program or a sibling of a before and after school child are eligible to attend Spring Break. Out of an abundance of caution, and to minimize COVID-19 risks, we are unable to accept care requests from those at another school or non-RisingOaks' families.

PAYMENT FOR SPRING BREAK
Spring Break care is offered at a pro-rated rate of $26.25 per day for children who attend RisingOaks' before and/or after school programs. For those who attend only before school or only after school, you will be invoiced for the difference between your before or after school rate and the $26.25 PA Day rate.

A sibling of a before/after school child may attend at a rate of $36.95 per day, for a minimum of 2-days.
